If there is a dental injury suffered while using a Shield Mouthguard, please call us at 800-828-7669, ext 309. We will forward you a claims kit. Please keep the packaging that the mouthguard came in to identify the mouthguard model and corresponding warranty.

Wearer may be covered by Shield’s warranty of $100.00 per injured tooth up to a maximum of $3,200.00.

Wearer may be covered by Shield’s warranty of $312.50 per replaced tooth and $250.00 per injured tooth up to a maximum of $10,000.00.

Wearer may be covered by Shield’s warranty of $468.75 per replaced tooth and $250.00 per injured tooth up to a maximum of $15,000.00.
